Is it possible to have extra shipping options set up at the check out? I've set up a flat rate shipping tiers but would like to add additional options in fixed fees such as insurance, express etc. for the customers to choose.
 
Any ideas?

Here is what I would do - there may be a better way:
 
Standard Shipping = $5.00
Express Shipping = $10.00
Standard with Insurance = $5.50
Express with Insurance = $10.50
 
All show at checkout as a list and the customer can select as required.
