When having available several shipping options (modules) like FEDEX and UPS and Custom... where can the customer actually select on how it should be shipped ? I do not see it during the checkout or anything.
 
I am planning on running the site from the US and the netherlands, so shipping is either from the US or from the netherlands, is it possible for the customer to solect where to ship from, or is there a way to make this automatic ? Shipping is always from default country, can that be made selectable (ship from US or ship from NL ?)

They usually appearing after user select delivery country. For FedEx and UPS user also need to specify their postal code.
Before using external modules like FedEx and UPS you need setup their settings.

Currently you can use only one default address but it's possible rewrite the modules to suit your specific needs.

The easiest option would be that the shipping modules would show up at checkout as a pull down selection. At this moment i have made 4 different shipping modules, but a customer can not select anything.
What would be needed for these shipping modules to shop up as being selectable at the checkout ?

I have it working so that the customer can choose at the checkout what kind of shipping (within my defined parameters) they want using radio buttons, I am sure you can change this to a drop down list in the code.
 
To do this, within each shipping module you must define a shipping method and set the parameters for it.
 
Once you have done that, if the customers order cost/weight/address... etc fall within one of those parameters the shipping method will show up at the checkout. If you have parameters that overlap, your customer will be given a choice of shipping methods.
