Description
When you enable Registration Notification to send the administrator an email after a user registers, there is an error on register page:
Notice: Undefined index: delivery_state_code in /home/public_html/includes/record.php on line 1
 
Warning: Cannot modify header information - headers already sent by (output started at /home/public_html/includes/record.php:1) in /home/public_html/blocks/block_user_profile.php on line 1168

Download the file above and extract 'block_user_profile.php' in /blocks folder of your shop replacing existing file. Please do not forget to make a backup copy of the current file in case something goes wrong.
